# HIV Risk Calculator: full reference Source: https://www.hivriskcalculator.com (generated 2026-10-29). All numbers below are produced by the same model as the calculator. Disclaimer: these are statistical estimates for education, not a diagnosis. Only an HIV test can confirm or rule out infection. Anyone with a possible exposure in the last 72 hours should seek PEP from a clinic or emergency department immediately. ## Key facts - PEP (post-exposure prophylaxis) must start within 72 hours of an exposure, ideally within 24 hours, and is taken daily for 28 days. CDC (2025) recommends follow-up HIV tests 4–6 weeks and 12 weeks after the exposure. - A lab antigen/antibody test (blood from a vein) is conclusive from 45 days after exposure; NAT from 33 days; rapid finger-prick and home self-tests from 90 days (CDC). - Undetectable = untransmittable (U=U): a person with HIV who keeps an undetectable viral load on treatment does not transmit HIV through sex. - PrEP taken as prescribed reduces the risk of getting HIV from sex by about 99%, and from injection drug use by at least 74% (CDC). - HIV is not transmitted by kissing, saliva, sweat, tears, sharing food or toilets, insect bites, or contact with unbroken skin. CDC rates biting, spitting and sharing sex toys as negligible. - CDC per-act estimates per 10,000 exposures to a source with HIV: receptive anal 138, insertive anal 11, receptive vaginal 8, insertive vaginal 4, needle-sharing 63, needlestick 23; oral sex is 'low' (0–4). - Circumcision lowers a man's risk of getting HIV through vaginal sex by 50–60%, but doesn't reduce transmission from a man who already has HIV. - Estimates from this site are statistical, not a diagnosis. Only an HIV test can confirm or rule out infection. ## How the estimate is calculated 1. Per-act risk for each exposure, assuming the source has HIV and isn't on effective treatment (CDC): - Receptive anal sex: 1.4% per act (138 per 10,000) - Insertive anal sex: 0.11% per act (11 per 10,000) - Receptive vaginal sex: 0.08% per act (8 per 10,000) - Insertive vaginal sex: 0.04% per act (4 per 10,000) - Oral sex: 0.01% per act (1 per 10,000) - Shared injection equipment: 0.63% per act (63 per 10,000) - Needlestick injury: 0.23% per act (23 per 10,000) - Blood on mucous membrane: 0.09% per act (9 per 10,000) - Kissing, bites/spitting, shared sex toys, fluids on unbroken skin: negligible. Several exposure types are combined as independent chances: 1 − (1 − p1)(1 − p2)… 2. Adjustments to per-act risk: condom −80%; no ejaculation −50% (receptive/oral); circumcised insertive partner −50%; another STI ×2; acute infection ×10; pregnancy ×2.82 and postpartum ×3.97 (receptive vaginal); needlestick deep injury ×15, visible blood ×6.2, needle from a vein/artery ×4.3 (odds, largest one used); partner undetectable: no sexual risk (U=U). 3. Chance the partner has HIV: 100% if known positive; 0% if a recent negative test is up to date; otherwise UNAIDS adult prevalence for their country, split by sex. A community figure (UK Black African; US Black or White) replaces the country figure. Key populations raise the odds: sex worker ×13.5, man who has sex with men ×23 (automatic for sex between two men), injects drugs ×14, trans woman ×48.8. A circumcised male partner (not MSM) is treated as ×0.5 as likely to have HIV, weighted by the country's circumcision rate. When several apply, the highest is used. 4. Treatment: for sexual exposures, only partners with HIV who aren't virally suppressed count. Suppressed share = country treatment coverage × 93%. 5. Repeated exposures: same partner → prevalence × (1 − (1 − p)^n); different partners → 1 − (1 − prevalence × p)^n. 6. PrEP: −99% (sex) / −74% (blood) when consistent; −90%/−50% with some missed doses; −50%/−20% if often missed. PEP: −90% within 24h, −80% at 24–48h, −70% at 48–72h; missed doses double the remaining risk. 7. Negative tests: Bayes' rule using the chance the test missed an infection on that day, falling from 90% at the start of the window to 0.5% at its end; conclusive after the window. Tests taken during PEP are ignored. - Nucleic acid test (NAT / RNA): detects from day 10, conclusive from day 33 - Antigen/antibody lab test (4th gen, blood from a vein): detects from day 18, conclusive from day 45 - Rapid antigen/antibody test (finger-prick): detects from day 18, conclusive from day 90 - Antibody test (most rapid tests and home self-tests): detects from day 23, conclusive from day 90 8. Risk levels: Very low (below 0.01%), Low (below 0.1%), Moderate (below 1%), High (1% or more). Each cell is the remaining chance after a single negative result on that day. "n/a" means the test can't detect HIV yet, so the risk is unchanged. A negative result after the window period is conclusive. | | Day 10 | Day 14 | Day 18 | Day 21 | Day 28 | Day 33 | Day 35 | Day 42 | Day 45 | Day 60 | Day 90 | |---|---|---|---|---|---|---|---|---|---|---|---| | Nucleic acid test (NAT / RNA) (window 10–33 days) | 1.2% (1 in 80) | 0.51% (1 in 200) | 0.21% (1 in 480) | 0.1% (1 in 950) | 0.022% (1 in 4,600) | 0.0014% (1 in 71,000) | 0.0014% (1 in 71,000) | 0.0014% (1 in 71,000) | 0.0014% (1 in 71,000) | 0.0014% (1 in 71,000) | 0.0014% (1 in 71,000) | | Antigen/antibody lab test (4th gen, blood from a vein) (window 18–45 days) | n/a | n/a | 1.2% (1 in 80) | 0.7% (1 in 140) | 0.18% (1 in 540) | 0.07% (1 in 1,400) | 0.048% (1 in 2,100) | 0.012% (1 in 8,000) | 0.0014% (1 in 71,000) | 0.0014% (1 in 71,000) | 0.0014% (1 in 71,000) | | Rapid antigen/antibody test (finger-prick) (window 18–90 days) | n/a | n/a | 1.2% (1 in 80) | 1% (1 in 100) | 0.61% (1 in 160) | 0.43% (1 in 240) | 0.37% (1 in 270) | 0.22% (1 in 450) | 0.18% (1 in 560) | 0.061% (1 in 1,600) | 0.0014% (1 in 71,000) | | Antibody test (most rapid tests and home self-tests) (window 23–90 days) | n/a | n/a | n/a | n/a | 0.85% (1 in 120) | 0.58% (1 in 170) | 0.49% (1 in 200) | 0.29% (1 in 350) | 0.23% (1 in 440) | 0.072% (1 in 1,400) | 0.0014% (1 in 71,000) | ### Chance that a partner of unknown status has HIV Before any treatment adjustment.
